Evaluation of Primary Care Interventions in Chronic Disease Management: A Longitudinal Multi-Center Study

Introduction
The escalating prevalence of non-communicable diseases (NCDs) globally, and specifically within South Asia, presents an urgent challenge to primary healthcare delivery. In Sri Lanka, general practitioners serve as the front line for managing conditions such as hypertension, type 2 diabetes, and COPD. Despite the accessibility of public healthcare, long-term management efficacy remains variable due to socio-economic factors and systemic bottlenecks.

This research seeks to isolate the impact of specific, low-cost primary care interventions that can be integrated into existing clinical workflows without requiring significant capital expenditure. By focusing on longitudinal outcomes, the study provides a clearer picture of sustainability in intervention benefits.
